In Home Primary Care
In Home Primary Care is a healthcare practice serving home bound elders in Snohomish County, WA. This practice grew from a concern that many elders in our community are no longer able to get to a provider’s office and thus are not receiving good or even adequate healthcare. Many are physically, emotionally or mentally unable to deal with the stress of leaving their home. In many situations there are no friends or family available to help.
In Home Primary Care provides home visits. We are a Nurse Practitioner practice. We do visits with our established patients routinely. If all is stable, we may visit once every 6-8 weeks. If there are any urgent needs or changes in condition, we try to do a next day visit. We are not an emergency service. Our goal is to help keep an individual stable, out of the hospital and the emergency department.
We visit patients in adult family homes, private homes and assisted living facilities. We do not go to Skilled Nursing Facilities.
We accept Medicare, Medicaid, most insurance plans except Group Health
